The Myopia Cascade

How myopia becomes a surgical emergency

Most Singaporeans know myopia as a glasses prescription. What they don't know is what happens inside a highly myopic eye over decades — and why catching it early changes everything.

Low Risk

Age 6–10

Myopia begins

Glasses. Normal life. Most parents think the story ends here.

Progressing

Age 10–20

Progressive myopia

Each dioptre stretches the eye ~0.35mm. At –8D, the eye is significantly elongated.

Caution

Age 20–40

Silent structural changes

Retina thins. Lattice degeneration. Risk accumulates invisibly.

Warning

Age 40–55

Early complications

Macular traction. Foveoschisis. 10.3% develop macular degeneration within 12 years.

Critical

Age 50+

Surgical emergency

Retinal detachment. Macular hole. Surgery within hours or days.

Protected

With Dr. Wong

Vision preserved

The earlier the relationship begins, the better the outcome at every stage.

Knowledge is reassuring. Dr. Wong believes in empowering patients and their families with clear, jargon-free information about their conditions and treatment options.

What is Retinal Detachment?

Emergency Condition · Surgery Required

The retina is the light-sensitive layer at the back of your eye. When it peels away from the tissue beneath it, this is called a retinal detachment — and it requires urgent medical attention.

  • Sudden floaters, flashes of light, or a "curtain" across your vision are warning signs
  • Most detachments are caused by a tear or hole in the retina
  • Surgery is highly effective, especially when treated early
  • If you notice symptoms, seek care immediately — do not wait

What is Macular Degeneration?

Age-Related Condition · Ongoing Management

Age-related macular degeneration (AMD) affects the macula — the central part of your retina responsible for sharp, detailed vision. It is the most common cause of vision loss in people over 60.

  • Dry AMD progresses slowly; wet AMD can cause rapid vision loss
  • Blurring, distortion, or dark spots in central vision are common symptoms
  • Anti-VEGF injections can significantly slow wet AMD progression
  • Regular monitoring is essential to catch changes early

What to Expect at Your First Visit

  • Avoid strenuous activity and air travel during recovery
  • Vision improvement can take weeks to months
  • Follow-up appointments are crucial for monitoring healing

I had vitrectomy done by Dr Wong to fix my retinal detachment, and continued my follow up with him at Asia Pacific Eye Centre. Dr Wong is a highly experienced, caring, and dedicated surgeon. Despite the very complex surgery (I had PVR), he patiently explained everything thoroughly, and made me feel very calm and assured. He had also given many helpful tips on pre and post surgery eye care, resulting in a good visual outcome. Will definitely recommend Dr Wong for his top notch skills and excellent service.
